I am an Osteopathic (Family) Doctor in Hawai’i, who personally had a spinal injury that led me to study posture and to Kathleen’s work.

that doesn't rely on overly developed muscles.

(including those who carry heavy loads on their heads).
While some people do suffer terribly from years of carrying heavy loads on their heads,
the outcome of this is determined by whether or not the weight of the load
has been distributed through aligned bones.

Currently, our society's remedy for chronic stiffness and muscle pain focuses almost solely on muscles.
We work to stretch tight ones and strengthen weak ones.
But we've missed the bigger picture:
Modern-day life has disrupted our essential underlying
framework of support (our bones), causing muscles,
that attach to these misaligned bones (via tendons),
to be either weak or tight in the first place.
A word about fascia:
Fascia is an overlooked yet vital connective tissue encapsulating our muscles, bones, blood vessels, and organs. This complex web essentially wraps our body parts together and, for our purposes here, fascia is considered part of the "musculo" component of our musculoskeletal system of pulleys and levers.
Fascia has nerves that contribute to pain sensations, most typically caused by skeletal misalignment. Even the deepest, most efficient fascial support is frequently undermined by misaligned bones in much the same way muscles are.
Disruption of our bony framework of support is at the very root
of an ever-growing, modern-day epidemic
of discomfort, pain, and other health problems related to this.
Common confusion about this drives the popularity of excessive muscle development
that's taken hold and often goes far beyond maintaining a healthy balance
between strength and flexibility that is inherent in our muscles' natural state of elasticity.
Luckily, we can learn from living role models
who show us a "new"* way forward.
* Except that it's actually as old as the hills.

Being sedentary is never a healthy option.
Regular movement is crucial to our health,
providing many benefits.
Luckily, there are many satisfying ways to move and stay active.
If you're lucky to live long enough . . .
the time will come when muscle strength and flexibility gained through repetitive strength training and stretching, will become more difficult to maintain. Without bones and muscles working harmoniously, you'll have to work harder and harder in an eventually futile attempt to prevent your body from gradually collapsing without the support of aligned bones.
The image below illustrates this progression. Exercise, in and of itself, doesn't predict how we age as much as HOW we inhabit our bodies while we exercise and move through our days.

We don't know if the elderly man in the brown shirt above was physically active in his younger years. However, this would not have prevented him from gradually collapsing because he's missing the foundational support of vertically aligned legs (the wrinkles in his pants reveal this). Without vertical legs, his pelvis "tucks" (like a sad dog tucking its tail)—disrupting the angle required to support an upright spine.
The man directly beneath him in the blue shirt is supported by solidly vertical legs and an elongated spine that rests on top of an aligned pelvis. This alignment keeps him on track to be comfortably upright throughout the years ahead. Ditto for everyone pictured to his left.

These images (left) reflect what we've tragically come to accept as "normal" aging. I came across the "Caution: Road crossing" sign (without words) near a retirement home in Portugal. It tells the heartbreaking story of how we've normalized structural collapse as if it were an inevitable feature of aging.
What makes this especially poignant is knowing these people could probably have avoided this fate. Had they understood the fundamental importance of a naturally positioned (anteverted) pelvis supported by vertical legs, their spines would have remained elongated, and their journey into old age is likely to have been dramatically different.
Now look closely at these elders (right). Their bodies reveal what lifetime alignment truly means: vertical legs that stand like solid pillars of support, elongated and supple spines, and joints that move freely, unencumbered by misalignment.
When I met the 93-year-old woman pictured squatting (bottom right), she was carrying a basket with an enormous load of bananas on her head. Later, she bent down to pick up something off the ground, and as you can see, she still bends the way she did when she was a toddler.
This woman has maintained the solid strength of her legs and the easy mobility of her knees and hips by bending this way for over ninety years. I'm pretty sure she has never "exercised" other than to live a physically active life, nor has she ever heard of a gym.

This isn't the artificial strength of acquired muscle.
This is the original power of our human design.
Such vital aging is the outcome of maintaining the body's natural blueprint—
kept alive in these people since childhood.
The good news is that long-forgotten movement patterns
can be re-learned, and mechanical strength
and fluid mobility regained.

The photos of these aligned older people were taken twenty-two years ago,
and while it's likely they have all passed on by now,

I hear this a lot when people first grasp these overlooked concepts. And of course,
it does make perfect sense that we humans who live in technologically advanced places
in the world, despite our many remarkable accomplishments, have lost our way.
We have forgotten our evolutionary design
and our inseparable physical relationship with the natural world,
of which we are all a part.
During the year I traveled to faraway places, I was continuously struck by stunning differences between the way local people inhabited their bodies compared to the tourists who visited their countries.

The images above tell a powerful story of contrasting skeletons. In the top row, we see bodies in harmony and partnership with gravity—relaxed, aligned, at ease— poetry in motion.
Below, we witness the all-too-familiar struggle against gravity's downward pull,
a battle marked by structural collapse, distorted muscles, and discomfort.
The solution to this might seem obvious:
stretch what's tight, and strengthen what's weak.
For one thing, this can feel good—temporarily—but soon we find
we are having to repeat stretching and/or strengthening
. . . without end.
Why?
Because when we're focused on our muscles (including fascia),
we're treating the symptoms and not the root cause of our problem.
Developing strength and stretching muscle tension have value in specific therapeutic situations, provided structural integrity is upheld, rather than targeting and isolating soft tissue any part of the musculoskeletal systemthat is meant to function as part of a whole system that affects multiple parts in everything we do.
Transforming your alignment requires you to go deeper
—bone-deep!—
and begin with putting your bones where they belong in relationship with each other.
It's not hard to do this—you just need to know HOW it's done.

⭐️⭐️ If you truly love your gym routine, with its reps and sets and all that comes with it, go right ahead and keep it! But do consider this: Your future comfort and health may be best served if you take the time to understand your body's natural design—the actual "form" you will need to apply to your gym workouts— and everything else you do.
"Building muscle" can be beneficial, especially as a way to head off sarcopenia as we age. But such benefits can be undermined without the support of aligned bones while working out
A more productive and sustainable goal in strength training at any age is to become naturally strong, not just bulky or "shredded." Otherwise, it's like painting over rust when you ignore the deeper (bone-deep) misalignment problem that often leads to common injuries, muscle imbalance, a dependence on workout sessions, and joint restriction and mobility problems down the road.
The benefits of movement throughout your life multiply when you're aligned while moving and exercising. Naturally aligned movement sustains itself in everything you do and gracefully carries you into old age.
